Job Summary: We are seeking a compassionate Nurse Practitioner (NP) to join our team as a House Calls Nurse Practitioner. As a key member of our healthcare team, you will provide evidence-based medical care to seniors and patients with complex needs in their homes or assisted living facilities. About the Role:
  • Examine and treat various health issues, managing patient records, and collaborating with other healthcare professionals to ensure comprehensive care.
  • Provide personalized care to seniors and patients with complex needs, promoting independence and quality of life.
  • Work collaboratively with caregivers and other healthcare professionals to maintain a comprehensive plan of care.
  • Stay up-to-date with industry developments, best practices, and regulatory requirements to deliver high-quality care.

Requirements:
  • Bachelor's degree in Nursing (BSN) or Master's degree in Nursing (MSN).
  • Active state NP license and current certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGNP).
  • At least 2 years of experience as a nurse practitioner, preferably in a similar setting.
  • Excellent interpersonal communication skills, verbal and written, with ability to engage with patients, families, and healthcare professionals.
  • Strong time management skills, with accountability for daily workflow and patient care.
  • Possession of valid automobile insurance and a reliable vehicle for travel to patient locations.
