Job Summary:
UCHealth is seeking a skilled Clinical Social Worker to join our team at UC Health Greeley Hospital. As a Clinical Social Worker, you will play a vital role in assessing patients and their families, caregivers, and legal representatives to arrange for necessary interventions.
Key Responsibilities:
- Assess patients and their support systems to identify needs and develop intervention plans.
- Provide short-term treatment planning for mental, emotional, and behavioral disorders, conditions, and addictions.
- Participate in case reviews to evaluate care management and progress toward goals.
- Assist with discharge planning and process.
- Identify and provide referrals for appropriate resources, including transportation, housing, healthcare, and social/spiritual services.
- Administer and interpret tests and measures of psychosocial functioning to diagnose mental, emotional, behavioral, addictive, and developmental disorders and disabilities.
- Supervise and train new staff, lower-level social workers, students, or interns.
Requirements:
- Critical thinking skills and decisive judgment.
- Ability to work with minimal supervision.
- Master's degree in Social Work.
