Healthcare Coordinator

We are seeking a skilled Healthcare Coordinator to join our team at Geisinger at Home. As a key member of our care coordination team, you will work closely with physicians and multidisciplinary team members to develop and implement personalized care plans for our patients.
Your primary responsibility will be to ensure that each patient receives high-quality, patient-centered care throughout their entire journey with us. This includes assessing patient needs, coordinating care with other healthcare professionals, and identifying potential barriers to effective care delivery.
Key Responsibilities- Work collaboratively with physicians, nurses, and other healthcare professionals to develop and implement comprehensive care plans
- Conduct thorough assessments of patient needs and preferences to inform care decisions
- C coordinate care services, including home health visits, medication management, and community resources
- Identify and resolve barriers to effective care delivery, such as communication breakdowns or lack of resources
- Maintain accurate and up-to-date records of patient interactions and care plans
This is a full-time position based in Bloomsburg, PA, with opportunities for remote work and flexible scheduling. We offer a competitive salary range of $62,000-$80,000 per year, depending on experience.

Requirements- 3+ years of experience in healthcare, preferably in a care coordination or case management role
- RN licensure in the state of Pennsylvania
- Basic Life Support certification
- Excellent communication and interpersonal skills
- Able to work independently and as part of a team
