Medical Professional for Long Term Care

Fusion Medical Staffing-Long Term Care is seeking a skilled medical professional to join their team in providing exceptional care to patients in Middlebury, Vermont.
Estimated Salary: $93,500 - $108,000 per year- Based on 8 hour shifts and 40 hours per week with tax-free stipend amount to be determined.
The ideal candidate will possess one to three years of experience as a Registered Nurse, with strong critical thinking, service excellence, and interpersonal communication skills. The ability to read and comprehend written instructions, follow verbal instructions, and operate a PC are essential.
- Other certifications may be required for this position.
The successful candidate will work collaboratively with physicians and other healthcare professionals to provide patient-centered care, administer medications as ordered, and maintain accurate documentation.
Key Responsibilities:- Communicate effectively with patients, families, and interdisciplinary teams.
- Develop and implement individualized care plans that address patients' physical, emotional, and social needs.
- Collaborate with healthcare professionals to coordinate patient care and ensure seamless transitions.
- Maintain accurate and timely documentation of patient progress, interventions, and outcomes.
- Administer medications as prescribed by physicians and maintain accurate medication records.
- Monitor patients' responses to treatments and interventions, reporting any concerns or changes to physicians and other healthcare professionals.
- Participate in ongoing education and training to enhance knowledge and skills in long-term care nursing.
