About the Role

We are seeking a dedicated and compassionate Field-Based Licensed Practical Nurse (LPN) to join our dynamic team at Author Health. As a Community Mental Health Nurse Specialist, you will provide essential nursing care in a variety of settings, including patients' homes and community-based environments.

Key Responsibilities
  • Deliver high-quality patient care, monitoring progress, and collaborating with interdisciplinary teams to support mental health treatment plans.
  • Conduct comprehensive assessments of patients' mental health status and medical status, track progress, and report any significant changes to the supervising psychiatrist or healthcare team.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Provide education to patients and their families about psychiatric conditions, medical conditions, treatment options, and medication management.
Requirements

To be successful in this role, you will need:

  • A valid LPN license and at least five years of experience as an LPN. Previous experience in psychiatric or mental health nursing is highly preferred.
  • Strong clinical assessment skills, excellent communication abilities, and proficiency in electronic health records.
  • The ability to work independently and manage time effectively in diverse and sometimes challenging environments.

  • An estimated salary of $65,000 - $85,000 per year, based on location and experience.
