Job Summary:
Care Coordinator Specialists are responsible for optimizing patients' recovery journeys. They complete weekly functional assessments, engage the PAC inter-disciplinary care team, and facilitate informed decisions with patients and families. This role requires strong clinical skills, effective communication, and a passion for delivering exceptional patient care.

Responsibilities:
• Perform Skilled Nursing Facility (SNF) assessments using clinical skills and measurement tools
• Review targets for Length of Stay (LOS), target outcomes, and discharge plans with providers and families
• Collaborate with healthcare teams to establish optimal discharges
• Participate in SNF Rounds providing accurate information to senior managers or medical directors
• Engage with patients, families, or caregivers weekly and as needed
• Assess and monitor patients' continued appropriateness for SNF setting according to InterQual criteria or nH Outcome

Requirements:
• Active, unrestricted registered clinical license in state of hire
• 5+ years of clinical experience working with adult or geriatric population
• Proficient with Microsoft Office applications
• Ability to work Monday to Friday between business hours of 7am to 6pm with weekly onsite travel
• Reliable mode of transportation and internet connection
