$96,390 - $120,490 per annum

Job Description

We are seeking a Statewide Travel Nurse Practitioner to join our Advanced Illness Management team. This role will report directly to the AVP of Advanced Illness Management (AIM) and involve facilitating advanced illness management and palliative care services for Empatia patients.

The responsibilities include:

  • Developing individualized treatment plans and providing comprehensive care from patients' homes, including diagnosis, treatment, consultation, and follow-up under the direction of the Palliative Medical Director or Primary Care physician.
  • Up to 80-90% statewide travel, requiring regular visits to palliative branches throughout the state to provide patient care and other essential responsibilities.
  • Potential occasional regional travel if needed, less than 10%.
  • Flexible or alternative schedules available.

Required Skills and Qualifications

To be successful in this role, you will need:

  • A Master of Science in Nursing from an accredited school or equivalent as allowed by state law.
  • At least 2 years of experience as a Nurse Practitioner.
  • A current license as a registered nurse and equivalent licensure required by state laws for Nurse Practitioner licensure.
  • An active and unrestricted Nurse Practitioner certification/license.
  • Multi-state licensure is desirable for Travel Nurse Practitioner roles or willingness to obtain additional state licensure (company-paid) for regional travel opportunities.
  • Practice experience with Part B documentation and coding.
  • A collaborative agreement with a supervising physician agreeable to supervise work in palliative care programs or a willingness to enter a collaborative agreement with the Palliative Medical Director, as required by state law.
  • Part B Provider certification.
  • A Federal DEA license.
  • A State DEA license or state-specific prescribing authority (if required).
  • ACHPN certification in Hospice and Palliative Care is preferred.
  • Nurse Practitioner specialty in Family or Adult Medicine, Acute Care is preferred.
