Job Title: Recovery Case Manager

About the Role:

We are seeking a highly motivated and compassionate Recovery Case Manager to provide individualized case management services to clients in our men's relapse prevention and transitional living program. As a key member of our team, you will work closely with clients to assess their needs, develop personalized treatment plans, and connect them with essential resources and services.

Key Responsibilities:
  • Provide direct program services to residents, utilizing cognitive behavioral techniques and strength-based approaches to support their recovery journey.
  • Conduct comprehensive screenings and assessments to identify client needs and develop effective case plans.
  • Coordinate individualized planning with clients to meet short- and long-term goals, including relapse prevention and housing plans.
  • Provide service linkages and support systems to ensure identified needs are met, and monitor progress towards agreed-upon goals.
  • Run assigned group therapy and educational sessions, utilizing any assigned curriculum to promote client engagement and understanding.
  • Document treatment planning, case records, charting, exit reports, and database reporting as required.
Requirements:
  • Bachelor's degree in a human services field, with a minimum of 1 year of experience in providing case management services to high-needs populations.
  • Knowledge of psychosocial treatment modalities, including cognitive behavioral therapies, motivational interviewing, relapse prevention, and social learning approaches.
  • Experience in assessing and developing goal-oriented service plans and intensive case management.
  • Strong knowledge and ability to work with issues related to homelessness, substance abuse, mental health diagnosis, and history of trauma.
  • Crisis intervention and non-violent mediation skills, with a commitment to treating all clients with dignity and understanding.
