The Role
As a Habilitation Assistant at ADAPT Community Network, you will work under general supervision, assisting in various program areas as assigned by the Program Director or designee. You will implement program plans, providing personal care to individuals receiving services, and assist in daily operations, supervision, and community outings.
Responsibilities
- Provide physical and programmatic aid to individuals where assigned.
- Aid in daily operation of assigned program areas.
- Assist with supervision of individuals in assigned areas.
- Accompany individuals on community outings, maintaining close supervision.
- Drive agency vehicles for transportation, pickups, deliveries, and medical appointments.
- Perform clerical work, including collating, folding, typing, and answering phones.
- Participate in case conferences and interdisciplinary planning.
- Collect data for written program plans, such as behavior and treatment plans.
- Provide necessary program support and coverage.
- Write case notes and report incidents as required.
- Follow established classroom activity schedules.
- Assist with toileting and feeding of individuals receiving services.
- Assist with outer clothing needs of individuals as required.
- Participate in activities outside the activity room, including trips, pool, gym, and swimming.
- Dispense prescribed medications after completing the medication administration course and following agency protocol.
- Attend supervisory conferences, staff meetings, and in-service training programs.
- Initiate precautions to ensure work is completed safely.
- Substitute in other groups as requested.
Requirements
- Valid New York State Driver's License and good standing.
- Ability to read, write, speak, and understand English.
- Good oral and written communication skills.
- Strong interpersonal skills for effective interaction with colleagues, employees, individuals, and their families.
- Ability to analyze problems and determine corrective measures.
Compensation and Benefits
You will be rewarded with a competitive hourly rate of $17.77-$19.06/hour, plus industry-leading benefits for all full-time employees.
