Job Summary

Mercy is seeking a full-time Nurse Navigator to provide expert care coordination for patients with cancer.

About the Role

The Nurse Navigator will work closely with patients, families, and healthcare teams to ensure seamless transitions throughout the continuum of care. This role requires strong clinical expertise, exceptional communication skills, and a passion for delivering compassionate care.

Key Responsibilities
  • Coordinate patient care across various settings, including hospitals, clinics, and homes.
  • Collaborate with interdisciplinary teams to develop and implement personalized care plans.
  • Provide education and support to patients and families on treatment options, side effects, and symptom management.
  • Facilitate communication between patients, families, and healthcare providers to ensure informed decision-making.
Requirements
  • Active RN licensure in the state of practice.
  • 2 years of nursing experience, preferably in oncology or a related field.
  • Excellent communication, critical thinking, and problem-solving skills.
