The Personal Care Specialist will provide individualized healthcare based on a care plan set by Pathwell Health to the elderly, convalescents, or persons with disabilities at the patient's home or in a care facility.



Key Responsibilities


  • Provide companionship and socialization to clients
  • Escort clients to recreational activities and medical appointments
  • Perform light housekeeping duties, as needed, to maintain a safe environment
  • Help clients with personal care needs, including bathing, dressing, and grooming
  • Assist clients with Activities of Daily Living (ADL's)
  • Teach and encourage self-care skills
  • Maintain complete and accurate documentation of clients' care
  • Maintain confidentiality in all matters related to the client's care


Requirements


  • One year of Caregiving experience (preferred)
  • Part-Time role available
  • HHA (preferred) or CNA (required), Driver's License, Car Insurance, Car Registration
  • Strong clinical assessment, organizational and communication skills
  • Ability to handle multiple assignments simultaneously
  • Excellent oral and written communication skills
  • Excellent customer relationship skills
