Job Summary:

Aveanna is seeking a skilled Physical Therapist Assistant to join our team in providing high-quality home health care services. As a Physical Therapist Assistant, you will work closely with our interdisciplinary team to deliver patient-centered care and achieve optimal outcomes.

Key Responsibilities:

  • Provide direct physical therapy services to patients in their homes, as directed by the physical therapist and in accordance with the physician's plan of care.
  • Communicate effectively with patients, caregivers, and healthcare team members to ensure seamless care coordination.
  • Conduct thorough assessments, develop and implement individualized treatment plans, and provide education to patients and caregivers on proper use of assistive devices and rehabilitation techniques.
  • Collaborate with other healthcare professionals to discuss patient progress, address concerns, and make adjustments to care plans as needed.
  • Participate in in-service programs, stay up-to-date with industry developments, and maintain professional certifications and licensure.

Requirements:

  • Graduation from a Physical Therapist Assistant education program from an accredited university/program.
  • Current, unrestricted state license as a Physical Therapy Assistant in the state of practice.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
  • Valid CPR and State Drivers License.
