Job Summary:We are seeking a skilled and compassionate Travel Nurse RN to provide high-quality patient care in our ambulatory care unit. As a Travel Nurse RN, you will be responsible for assessing and evaluating patient care needs, developing and implementing individualized care plans, and collaborating with the interdisciplinary healthcare team to ensure seamless patient care.
Responsibilities:- Assess and evaluate patient care needs, applying critical thinking skills to develop and implement individualized care plans.
- Deliver patient care by incorporating the tenets of Relationship Centered Care and through the application of the nursing process.
- Collaborate with the interdisciplinary healthcare team to ensure seamless patient care and effective communication.
- Manage assignments within the Care Delivery Model of Modified Primary Nursing, demonstrating effective allocation of material resources.
- Educate patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease.
- Communicate effectively and professionally with patients, family, and all members of the Interdisciplinary Patient Care Team.
- NYS RN/LPN License
- BLS
- 2 years of ambulatory experience
- Minimum 2 years RN/LPN experience
- EPIC experience highly preferred

Additional Details:- Performs charge nurse duties as assigned.
- Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager to ensure supplies are appropriately utilized and inventory is sufficient for patient care.
- Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Shift Manager.
- Demonstrates patient-focused and cost-effective approaches to patient care in terms of equipment, staff, supplies, and all other resources.
- Documents in an accurate and thorough manner in compliance with hospital, regulatory, and legal requirements and standards of care.
- Facilitates patient throughput on shift basis.
