Job Overview

Lund, a multi-service nonprofit organization, is seeking a skilled Substance Use Case Manager to join its Regional Partnership Program in Newport. This rewarding role involves partnering with DCF Family Services and supporting caregivers impacted by substance use.

About the Role

The ideal candidate will possess strong problem-solving skills, excellent communication abilities, and a passion for working with families and multidisciplinary teams. As a key member of the intake unit, you will conduct substance use screenings, coordinate referrals to assessment and treatment, and support treatment engagement by addressing barriers. Your expertise will also be sought in providing consultation and training for DCF staff on topics related to substance use and treatment services.

Responsibilities
  • Provide comprehensive case management services to families affected by substance use
  • Conduct thorough assessments and develop individualized care plans
  • Collaborate with treatment providers and community resources to ensure seamless service delivery
  • Offer consultation and training to DCF staff on substance use and treatment best practices
Requirements
  • Bachelor's degree in Social Work, Counseling, or a related field
  • Experience in case management, preferably in substance use or mental health settings
  • Apprentice Addiction Professional (AAP) certificate or ability to obtain within first year
  • Excellent communication, interpersonal, and problem-solving skills
