Job Description

We're looking for a Licensed Practical Nurse who shares our values of being Trustworthy, Capable, Compassionate, Proactive, and most importantly, Called to serve patients in their home.

The ideal candidate will work in collaboration with the RN to fulfill the defined patient-specific care plan, collaborate with the care team to ensure all patient needs are fully addressed, deliver high-quality skilled care to patients, thoroughly document care delivery daily in our EMR system, and contribute to a culture of caring through individual accountability and teamwork.

Requirements

To succeed in this role, you'll need to be compassionate in care delivery, focused on results, solution-driven, self-motivated, and responsive to urgency. You should also be comfortable with technology and have experience working independently without direct oversight.

Able to discern when to call for support and communicate challenges, familiar with technology, HCHB experience is a plus.

Compensation

Salary range: $55,000 - $75,000 per year, depending on location and experience.
