Optum CA is seeking a highly skilled Nurse Practitioner or Physician Assistant to join its team in Ontario, CA.

About the Role:

This exciting opportunity will see you work as an integral part of Optum's vision to improve healthcare for everyone. You will be working alongside talented peers in a collaborative environment guided by diversity and inclusion, driving towards the Quadruple Aim.

Your Responsibilities:

  1. Serve the Southwestern Valley region
  2. Complete senior wellness visits in clinics, virtually, and SNF/custodial settings to see 10-14 patients per day
  3. Travel to Optum outpatient clinics and independently contracted physician offices, as well as some home and SNF custodial visits, 1-2 locations per day
  4. Conduct comprehensive assessments, perform recommended screenings (such as QuantaFlo, Neurometrix, or point-of-care A1C), comprehensively document and code encounters, conduct patient education, and follow up
  5. Work a schedule of Monday - Friday, with occasional need for Saturday clinics

Requirements:

  • Master's degree from a four-year college and/or professional certification beyond a four-year college
  • Certificate of completion from an accredited Advanced Practice Clinician Program
  • Unrestricted California Advanced Practice Clinician license
  • Current AANP/ANCC/NCCPA certification
  • Current California DEA certificate required prior to start date
  • Fluent or proficient Medical Spanish
  • EMR Proficient
  • Excellent patient care and time management skills
  • Ability to work in a fast-paced environment, team player, adaptable to changing priorities
