Position Summary:
The Patient Care Coordinator plays a pivotal role in ensuring seamless patient services by scheduling appointments and coordinating with staff members. Position Specific Responsibilities:
- Schedule and document patient services in a timely manner.
- Collaborate with management and the interdisciplinary team to coordinate care.
- Communicate effectively with patients, staff, and healthcare professionals.
- Respond professionally and creatively to agency and patient needs.
- Perform on-call duties and other tasks as assigned.
Education and Experience:
- A high school diploma or equivalent is required.
- Prior experience with patient scheduling is preferred.
- Customer service experience is also preferred.
Skills:
- Proficient in basic math, including calculating rates and percentages.
- Strong written and verbal communication skills.
- Ability to read and interpret business and technical documents.
- Fluent in English, with a strong commitment to the integrated healthcare philosophy.
