Position Summary:

The Patient Care Coordinator plays a pivotal role in ensuring seamless patient services by scheduling appointments and coordinating with staff members. Position Specific Responsibilities:

  • Schedule and document patient services in a timely manner.
  • Collaborate with management and the interdisciplinary team to coordinate care.
  • Communicate effectively with patients, staff, and healthcare professionals.
  • Respond professionally and creatively to agency and patient needs.
  • Perform on-call duties and other tasks as assigned.

Education and Experience:

  • A high school diploma or equivalent is required.
  • Prior experience with patient scheduling is preferred.
  • Customer service experience is also preferred.

Skills:

  • Proficient in basic math, including calculating rates and percentages.
  • Strong written and verbal communication skills.
  • Ability to read and interpret business and technical documents.
  • Fluent in English, with a strong commitment to the integrated healthcare philosophy.
