Job Description

We are seeking a skilled Healthcare Manager to join our team at Holistic Health. As a key member of our care coordination team, you will play a vital role in ensuring the well-being of children with medical or mental health diagnoses.

Your primary responsibility will be to connect these individuals with the necessary providers and specialists, while coordinating with their care teams to achieve the best possible outcomes.

A crucial aspect of this role is conducting timely assessments to identify members' and families' needs. Based on these findings, you will create personalized 'plans of care' that address their unique requirements.

This includes but is not limited to:

  • Housing
  • Physical and mental health
  • Education
  • Food security
  • Dental
  • Activities of daily living

To excel in this position, you must be able to:

  • Conduct assessments efficiently
  • Coordinate services effectively
  • Maintain accurate documentation of progress
  • Establish regular communication with members, families, and care teams
  • Ensure members' profiles remain up-to-date

The ideal candidate will possess a Bachelor's degree, 2 years of relevant experience, strong writing skills, and basic computer proficiency.

Estimated Salary: $60,000 - $80,000 per year
