Palliative Care Social Worker

Avera Medical Group is seeking an experienced Palliative Care Social Worker to join our team in Yankton, South Dakota. As a key member of our clinical care team, you will play a vital role in providing social work services to patients and families facing chronic illness.
The ideal candidate will possess a Master's degree in Social Work from an accredited program and be licensed as a Social Worker in the State of South Dakota. A Qualified Mental Health Professional Credential is also highly preferred.
About the Role- You will assess the needs of patients facing chronic illness and facilitate plans with patient/family to promote effective care.
- Maintain a current knowledge base of available community agencies and resources to facilitate appropriate referrals.
- Provide direct clinical therapeutic services to patients as part of an overall comprehensive patient care plan.
- Competitive hourly rate of $28.75-$43.50 per hour (dependent on experience).
- PRN scheduling options to fit your lifestyle.
- Master's degree in Social Work from an accredited program required.
- Licensed as a Social Worker in the State of South Dakota required (multi-state licensure may be required).
- Qualified Mental Health Professional Credential (in South Dakota) preferred.
