Job Summary

UPMC Home Health is seeking a full-time Physical Therapist to join our team. As a Physical Therapist, you will be responsible for completing SOC assessments in Allegheny and Westmoreland County. This is a float position, meaning you will work a total of 36 hours a week, with three 12-hour days. You will need to be available Monday through Friday during the orientation period.

Responsibilities
  1. Perform duties and job responsibilities in a fashion that coincides with the service management philosophy of UPMC Home Healthcare towards patients, visitors, staff, peers, physicians, and other departments within the organization.
  2. Continually develop professional skills by assuming responsibility for ongoing continuing education.
  3. Participate in on-call coverage rotation for the agency.
  4. Perform by system-wide competencies/behaviors.
  5. Assign and supervise the Physical Therapy Assistant according to the guidelines of the State Board of Physical Therapy.
  6. Advise and consult with the family, inter-intradisciplinary team members, physicians, and other professionals, as appropriate, to ensure optimal patient care.
  7. Data syncs (transmits) information same day.
  8. Maintain and complete all documentation promptly and accurately, including visit assessment, OASIS, 485, and Verbal Orders on the laptop device or in written documentation while in the patient's home and within the agency and regulatory standards.
  9. Establish the plan of care in collaboration with the physician, patient, and UM professional by the physician's orders and patient needs and follows the admission process.
  10. Evaluate, plan, and treat patients in an effective physical therapy program utilizing diagnostic, muscle, nerve, joint, and functional ability testing within the standards of Physical Therapy Practice.
  11. Makes recommendations and orders necessary DME when indicated.
