We are seeking a highly skilled Wound Care Coordinator to join our team at Brooklyn Center in Brooklyn, NY. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional wound care services to our patients.

About the Role

The successful candidate will be responsible for supplementing wound care management for assigned centers, conducting weekly wound rounds with wound care consultants/providers, and ensuring timely MD and family notifications and follow-ups. Additionally, they will complete weekly skin monitoring and document it in PCC, review new admissions and readmissions for accurate wound information, and make recommendations for pressure ulcer prevention interventions.

Qualifications and Requirements

To be considered for this role, you must have a nursing degree from an accredited college or university, current State License as a Registered Nurse, and experience in long-term care settings. A background in wound care certification is highly preferred.
