Acute Care Case Manager

Mass General Brigham is seeking a skilled Registered Nurse to fill the role of Acute Care Case Manager. This position involves coordinating post-acute care services for patients admitted to Home Hospital, ensuring a seamless transition from hospital to home.
Key Responsibilities- Develop and implement discharge plans for patients requiring post-acute care services
- Collaborate with patients, families, caregivers, and healthcare teams to ensure safe and effective discharge
- Conduct ongoing assessments of patients' health status and revise discharge plans as needed
- Ensure timely and accurate insurance reviews and approvals for post-hospital care
- Participate in quality and risk management case finding activities

Requirements
- Graduate of an accredited school of nursing
