About the Role

As an Advanced Care Coordinator at Tenet Healthcare Corporation, you will play a pivotal role in delivering high-quality patient care. Your responsibilities will include coordinating care activities, establishing strong relationships with physicians and other healthcare providers, and delegating patient care to ensure seamless delivery.

Main Responsibilities
  • Collaborate with multidisciplinary teams to develop and implement comprehensive care plans
  • Provide expert-level coordination of patient transfers and discharges
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making
  • Maintain accurate and detailed records of patient interactions and care activities
Requirements
  1. Bachelor's degree in Nursing (BSN) or equivalent experience
  2. Licensed Registered Nurse (RN) in Michigan
  3. Current American Heart Association (AHA) Basic Life Support (BLS) certification
  4. Active ACLS certification
