Job Summary:

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Home Health Nurse - Geriatric Care Specialist. In this role, you will be responsible for providing expert nursing care to patients in their homes, promoting independence, and enhancing their overall well-being.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' medical needs and develop individualized care plans to address their unique requirements.">
  • Deliver high-quality nursing care, ensuring timely interventions and effective coordination with interdisciplinary teams.">
  • Foster open communication with patients, families, and caregivers, providing education and support to promote informed decision-making.">
  • Collaborate with physicians and other healthcare professionals to ensure seamless transitions and optimal patient outcomes.">
  • Monitor and manage medication regimens, including administration, monitoring, and discontinuation as ordered by physicians.">
  • Participate in regular team meetings to discuss patient progress, share best practices, and address any concerns or challenges.">
  • Maintain accurate and thorough documentation of patient interactions, including progress notes, summaries, and discharge reports.">
  • Utilize electronic medical records and documentation software to ensure efficient data management and compliance with regulatory requirements.">

Requirements:

To succeed in this role, you will need:

  • ">
  • A current unrestricted registration and license in the applicable state, with a minimum of one year of nursing practice experience in a patient care setting, preferably in home health, geriatrics, or related fields.">
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities in a fast-paced environment.">
  • Excellent communication and interpersonal skills, with the capacity to build rapport with patients, families, and healthcare colleagues.">
  • Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration's policy.">
  • Current CPR certification.">
