Job Overview

We are seeking a skilled Case Manager to join our team at Day One, where you will play a vital role in providing comprehensive case management services to individuals in the Southern Maine region.

The primary responsibilities of this position include developing, implementing, and monitoring service plans; working collaboratively with a team to achieve client goals; identifying and coordinating necessary services; and maintaining accurate records.

Key Responsibilities
  • Service Planning: Develop and implement individualized service plans that address the unique needs and goals of each client, ensuring seamless coordination with community resources.
  • Collaboration and Coordination: Work closely with clients, families, and healthcare providers to ensure effective care delivery and timely interventions.
  • Case Management: Conduct regular assessments, develop and update service plans, and coordinate services to meet the changing needs of clients.
  • Documentation and Reporting: Maintain accurate and up-to-date records, including progress notes, service plans, and statistical data, to support program evaluation and quality improvement initiatives.
Requirements and Qualifications
  • MHRT-C Certification
  • LSW or LSW preferred
  • Experience working with individuals with mental health and/or substance use concerns
  • Excellent communication, interpersonal, and problem-solving skills
  • Able to work independently and as part of a team
  • Reliable transportation and ability to travel locally
