About the Position

We are seeking a skilled Caregiver Specialist to join our team at Ontario Center for Rehabilitation and Healthcare in Canandaigua, NY.

Job Summary

The ideal candidate will have a Bachelor's Degree in Human Services, Psychology or Social Work, with long-term care/subacute experience required. As a Caregiver Specialist, you will be responsible for maintaining and improving residents' physical and psychosocial needs, identifying individual social and emotional needs, developing and maintaining individualized care plans, and participating in interdisciplinary care plan meetings.
