About the Role

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Abode Care Partners. As a House Calls Nurse Practitioner, you will provide high-quality medical care to seniors and patients with complex needs in their homes and community settings.

Responsibilities
  • Plan, direct, and evaluate patient medical needs, providing evidence-based care.
  • Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other health issues.
  • Collaborate with caregivers and other healthcare professionals to ensure comprehensive care plans.
  • Order and interpret laboratory and diagnostic testing as needed.
  • Provide referrals to other healthcare practitioners when necessary.
  • Maintain accurate and up-to-date patient medical records.
Requirements
  • A valid state Nurse Practitioners license.
  • Current Nurse Practitioner National Board Certification.
  • An active state DEA licensure.
  • A Master's degree in Nursing (MSN).
  • A minimum of five years of nursing experience, with at least two years of nurse practitioner experience preferred.
  • Excellent interpersonal communication skills – verbal and written.
  • Strong time management skills with accountability for daily workflow.
  • Proficiency in medical instruments and equipment.
  • Demonstrated knowledge of HIPAA and OSHA regulations.
  • Ability to engage in moderate physical activity.
