We are seeking a dedicated and compassionate individual to join our team as a Full-Time Resident Care Coordinator at Community of Hope. As a key member of our housing program, you will play a vital role in ensuring the safety and well-being of our residents, while also providing exceptional support services to help them achieve their goals.

Our comprehensive approach focuses on healthcare, housing, and supportive services, with a strong emphasis on maternal and child health, behavioral health, and community development.

Job Summary:

The Full-Time Resident Care Coordinator will be responsible for monitoring the building, enforcing resident rules, serving as a part of the emergency response team, and communicating with case managers to ensure that clients receive the necessary support services. This role requires a regular shift from 4pm-12am, Friday-Monday, and may involve working evenings and weekends.

Key Responsibilities:

  • Monitor community areas within the building, including the lobby, phone room, and exterior areas, to prevent loitering and maintain a safe environment.
  • Respond to emergencies and incidents, using program staff and police as backup when necessary.
  • Communicate effectively with residents, staff, and external agencies to ensure seamless support services.
  • Maintain accurate records and documentation, including incident reports and shift logs.
  • Collaborate with case managers to develop and implement individualized plans for residents.

Requirements:

  • High school diploma or GED required.
  • Excellent communication and interpersonal skills.
  • Punctuality and reliability essential.
  • Ability to work effectively in a fast-paced environment.
  • Basic computer skills, including Microsoft Office and Google Mail.
