The Nurse Coordinator for Oncology Multispecialty Care Centers is an integral member of an assigned Disease Team. The OMCC Nurse Coordinator focuses on: navigating the patient and family across care settings according to the established patient care plan; ensuring that the patient/family have appropriate information, understand and have the capacity to adhere to the medical plan; and bridging between the various consultants, services, points of care encountered by patient/family.

Main Responsibilities:

  • Navigating patients and families across care settings according to the established patient care plan.
  • Ensuring patients and families have necessary information and can adhere to the medical plan.
  • Bridging between various consultants, services, and points of care.
  • Serving as a primary contact for new patients and their families.
  • Partnering with Intake Assistants to expedite entry of new patients into the system.
  • Collaborating with Disease Team members to ensure coordination of care across the continuum.

Educational Requirements:

A Bachelor's Degree in Nursing is highly preferred; a Master's Degree in Health-related field is preferred.

Experience:

A minimum of three (3) years experience in adult inpatient services, ambulatory oncology service or operating room. Oncology experience is preferred.

Licensure:

Connecticut RN license is required.

Certification:

Oncology Nursing Society certification (OCN, AOCN) is desirable.

Estimated Salary:

$115,000 - $140,000 per year.
