Geisinger at Home offers an exciting opportunity for a skilled Nurse - Care Coordinator to join our team. With a strong focus on delivering exceptional patient care, we are looking for a dedicated individual who can work collaboratively with physicians and multidisciplinary teams to develop personalized plans of care for each assigned patient. As a key member of our care coordination team, you will play a vital role in ensuring seamless transitions between healthcare settings and facilitating timely discharges or alternative plans of care that meet the needs of patients, families, and payers.

Key Responsibilities:
  • Collaborate with healthcare professionals to assess, plan, and initiate patient plans of care, incorporating evidence-based practices and standardized guidelines.
  • Facilitate communication among healthcare providers, patients, and families to ensure accurate information exchange and resolution of issues affecting care delivery.
  • Develop and implement individualized care coordination plans, addressing physical, emotional, and social needs of patients and their support systems.
  • Monitor quality of care using established criteria, reporting potential risks and quality management concerns to appropriate departments through defined processes.
  • Capture data as required for quality improvement initiatives and contribute to ongoing case reviews for delays or variances in care.


About Geisinger at Home: Our innovative care model brings personalized healthcare services directly to eligible Geisinger Gold Medicare Advantage members within their homes. This position requires extensive travel (more than 50%) between patient locations within Columbia County, PA, and occasional visits to Danville for meetings and coverage of surrounding counties. A minimum of three years of RN experience is required, with home health experience preferred but not essential. Requirements and Qualifications:
  • Graduate from a Specialty Training Program-Nursing (Required).
  • Minimum of 3 years of Nursing experience (Required).
