We are seeking an experienced LVN Home Health professional to join our team as a Nurse Care Specialist. This role is responsible for providing high-quality patient care in the home setting, working collaboratively with interdisciplinary teams to ensure optimal outcomes.

The ideal candidate will have a strong background in nursing, with at least 1 year of experience in Med/Surg or acute hospital settings. A State licensed LVN with proficiency in intermediate nursing skills, clear effective written and oral English skills, and objective active listening skills is required. The ability to work independently within the confines of educational preparation, physician orders, and as assigned and supervised by a registered nurse is essential.

Estimated Salary: $65,000 - $85,000 per year

Key Responsibilities:

  • Deliver high-quality patient care in the home setting, adhering to established protocols and procedures.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Conduct comprehensive assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
  • Provide education and support to patients and their families on disease management, medication adherence, and self-care techniques.
  • Document patient progress, identifying trends and patterns that inform care planning and decision-making.
