We are seeking a skilled Patient Care Coordinator to join our team at Ascension Living. As a key member of our healthcare team, you will provide direct nursing care to patients in the long term care setting and implement patient care plans tailored to individual needs.

Job Summary

The ideal candidate will possess excellent communication skills and be able to work effectively with interdisciplinary teams to coordinate patient care. This is an exciting opportunity to make a meaningful difference in the lives of our residents and their families.

Based on industry standards and cost of living in Wichita, KS, we estimate the salary for this position to be around $34.10 per hour. However, actual compensation may vary based on factors such as education, experience, and qualifications.

Responsibilities
  • Provide direct nursing care to patients in accordance with established policies and procedures
  • Implement and monitor patient care plans to ensure optimal outcomes
  • Coordinate with interdisciplinary teams to deliver well-rounded care
  • Note and carry out physician and nursing orders in a timely manner
  • Identify and address safety hazards to ensure resident well-being
Requirements
  • BLS Provider certification within 1 month of hire date required
  • Registered Nurse license obtained prior to hire date required
  • Diploma from an accredited school/college of nursing required
