Corewell Health's Ambulatory Care team is dedicated to providing high-quality, patient-centered care in our outpatient clinics across Michigan. As a key member of our team, you will play a vital role in promoting optimal health outcomes for our patients.

Scope of Work

As an Ambulatory Care Manager, you will be responsible for planning and prioritizing care for individuals and populations of patients. You will demonstrate expertise in nursing care and management of a caseload of patients with varying complexity, and seek to improve patient, family, and health systems/community outcomes through the application of educational concepts/skills and preventive care in a managed care environment.

  • Assess internal and external referrals to identify patient/significant others' needs, level of intensity, insurance benefits, and other patient resources.
  • Develop a plan of care and make recommendations to PCPs, specialists, and other members of the healthcare team regarding care management strategies, identifying strategies to maximize continuity of care across the continuum.
  • Assess the educational needs of patients, families, and members of the healthcare team and develop and implement appropriate teaching strategies or make appropriate referrals.
  • Communicate and collaborate with patient/significant others/providers/payers to coordinate services that improve access to appropriate services across the continuum of care and which promotes optimal health in a cost-effective manner.
  • Measure patient care outcomes, interpret reports, and analyze data trends for groups of patients.
  • Document patient data, plan, interventions, and outcomes according to department guidelines.

Qualifications
  • Required: Bachelor's Degree in Nursing, Registered Nurse - State of Michigan License, 2 years of relevant experience in clinical nursing and/or case management/managed care or related field.
  • Preferred: 5 years of relevant experience in clinical experience.
