Job Description:

We are seeking a skilled Care Counselor to join our team at the Karen Ann Quinlan Memorial Foundation. As a Care Counselor, you will be responsible for providing psychosocial assessments and developing care plans that meet the needs of individual patients and families.

Key Responsibilities:

  • Perform thorough psychosocial assessments within 5 days of admission.
  • Develop and implement effective care plans that address patient and family needs.
  • Solve routine care plan problems independently.
  • Advocate for services from community organizations as needed.
  • Provide emotional support through short-term counseling sessions.
  • Meet established goals in a timely manner.
  • Presentation of accurate information in internal and external meetings.
  • Maintain accurate records of visits, phone calls, and meetings.
  • Provide social work counseling and follow-up visits or phone calls as needed.
  • Participate in interdisciplinary team meetings and conferences.
  • Comply with employment policies, maintain confidentiality, and adhere to HIPAA regulations.

Position Requirements:

The ideal candidate will possess knowledge of the dying process, policies, and procedures of the hospice organization. They should also have expertise in regulations surrounding compliance with Medicare Conditions of Participation. Additionally, they will need to demonstrate excellent interpersonal and communication skills, both written and verbal.

Preferred Qualifications:

  • One year of post-Master's Social Work or counseling experience.
  • Prior experience in a healthcare setting is preferred.

Salary and Benefits:

This full-time position offers an estimated salary of $60,000 per year, based on the location in Warren County, NJ. The role is exempt and requires working a standard shift schedule. We offer competitive benefits, including opportunities for professional growth and development.
