Job Title: Treatment Coordinator/QIDP
Job Summary:

AHRC New York City is seeking a dedicated and compassionate Treatment Coordinator/QIDP to join our team.


Job Description:

The Treatment Coordinator/QIDP is responsible for coordinating the training and habilitation program of assigned residents with residential staff, educational and vocational rehabilitation programs, and other needed services.


Key Responsibilities:

  1. Serve as a member of the interdisciplinary team to coordinate integration of various services in planning, implementation, and evaluation of an individual's treatment plan.
  2. Generate Needs/Capabilities lists annually, with updates as needed, ensuring each assigned client has an activity schedule, ongoing continuous active treatment, and implementation of an activity schedule as documented.
  3. Act as resident advocate in areas related to resident cleanliness, ensuring adequate clothing, personalized and clean bedrooms, toiletries, etc.
  4. Complete reviews of each assigned resident's individual treatment plan by summarizing progress or need for program modification as supported by concrete, measurable data obtained by direct care staff.
  5. Act as liaison with day/workshop programs, with ongoing face-to-face and phone contact with day providers, ensuring coordination of services between day and residential components.
  6. Ensure that IPPs and day program progress notes are received in timely fashion and remain on file.
  7. Ensure that direct care/clinical staff are trained in behavioral interventions and active treatment areas, so all staff working with individuals can carry out treatment plans as written.
  8. Coordinate Annual Review process by ensuring reports from all disciplines are collected in timely manner, preparing annual report, and generating goals, objectives, and interventions for indicated needs.

Qualifications:

  • Bachelor's Degree in a human services-related field.
  • Basic knowledge of all clinical areas that comprise the interdisciplinary team for individuals with intellectual disabilities.
  • Good working knowledge of direct care responsibilities and ability to perform such duties.
  • Leadership abilities and ability to coordinate a team of clinical and management staff.
  • Excellent organizational skills and good writing ability.
  • Ability to deal tactfully, professionally, and effectively with residents, personnel, family members, visitors, government agencies, and the general public.
  • Ability to work flexible hours.
