Overview

Catholic Health Initiatives, a nonprofit Catholic health system, is seeking a Family Medicine Advanced Practice Provider to join our established practice in Omaha, Nebraska. Key Responsibilities

  • Provide high-quality, patient-centered care in an outpatient setting
  • Collaborate with our multidisciplinary team to ensure seamless care coordination
  • Manage a patient panel with an average of 20-24 patient visits per day
  • Develop and implement individualized care plans to meet patient needs
  • Participate in quality improvement initiatives to enhance patient outcomes

Qualifications

  • Master's degree in Family Nurse Practitioner or Physician Assistant from an accredited program
  • Minimum one year of experience as a Nurse Practitioner or Physician Assistant
  • Nebraska license and DEA certification required
  • Strong communication and interpersonal skills
