Job Summary

We are seeking a highly skilled Registered Nurse to join our Referral Coordination Team. As a key member of our team, you will be responsible for coordinating referrals, providing exceptional patient care, and working collaboratively with healthcare professionals to ensure seamless transitions of care.

Key Responsibilities
  • Coordinate referrals and admissions for patients, ensuring timely and efficient transitions of care
  • Provide comprehensive patient assessments, develop individualized care plans, and implement evidence-based interventions
  • Collaborate with healthcare teams to ensure continuity of care, address patient concerns, and resolve issues in a timely manner
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal outcomes
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies
Requirements
  • Current RN licensure in the state of practice
  • Minimum 2 years of experience in a healthcare setting, preferably in a referral coordination or case management role
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities
  • Proficiency in electronic health records and other relevant software systems
