Home Health Nurse Position with Excellent Benefits

Schedule and Compensation
This position offers flexible scheduling options with a minimum of 1 scheduled weekly hour. The compensation range for this role is estimated to be between $80,000 - $110,000 per year, based on starting base pay for full-time employment at 40 hours per week. Pay will vary based on demonstrated job-related skills, knowledge, experience, education, certifications, etc.
Job Description
As a Home Health Registered Nurse, your primary responsibilities will include:
* Providing admission, case management, and follow-up skilled nursing visits for home health patients.
* Administering ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
* Conferencing with physicians in developing the initial plan of treatment based on physician orders and initial patient assessments. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision. Revise the plan in consultation with physicians based on ongoing assessments and as required by policy/regulation.
* Coordinating appropriate care, encompassing various healthcare personnel such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
* Reporting patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis.
* Implementing patient care plans in conjunction with patients and families to assist them in achieving optimal resolution of needs/problems.
Required Skills and Qualifications
To be successful in this role, you will need:
* Diploma, Associate, or Bachelor Degree in Nursing
* Minimum of one year nursing experience preferred
* Strong med surg, ICU, ER, acute experience
* Home Health experience a plus
* Current and unrestricted Registered Nurse licensure
* Current CPR certification
* Strong organizational and communication skills
* Valid driver's license, auto insurance, and reliable transportation.
