Nurse Care Manager

We are seeking a skilled and compassionate Registered Nurse to join our team as a Nurse Care Manager in our Group Home Services. As a key member of our care team, you will be responsible for ensuring high-quality patient care and providing leadership in clinical planning and coordination.
Key Responsibilities:- Review patient charts for missing labs, reports, and open orders
- Triage urgent same-day or double-book appointment requests
- Coordinate and participate in daily huddles
- Assist with inventory management
- Prepare equipment and aid physicians during treatment, examination, and testing of patients
- Verify chief complaint and take vital signs as requested
- Review the electronic health record (EHR) active medication list with patients
- Conduct and document point-of-care tests based on protocol
- Perform injections as directed by providers
- Provide general nursing care to patients, including administering prescribed medications and treatments in accordance with nursing standards
- Plan, articulate, and coordinate care and interventions
- Maintain consideration for specific age-related characteristics, needs related physical development, communication, and socialization, and safety, in collaboration with other members of the care team
- Assist with COVID-19 testing and vaccination processes
- Facilitate clinical messages and chronic condition management education
- Provide abnormal test results to patients and offer recommendations based on provider determination
- Add recommendations to individualized care plans and review After Visit Summaries (AVS) with patients
- Document progress notes in the electronic health record within 24 hours
- Manage patient flow through primary care clinics
- Perform CLIA waived tests as needed
- Maintain all environment of care quality logs
- Participate in scheduled clinic rounds to maintain the environment of care
- Conduct medication management, including performing medication reconciliation, reinforcing medication knowledge, and understanding medication regimens; refer to pharmacists as required
- Enforce adequate Infection Control standards, with guidance from Medical Directors
- Must be a Registered Nurse licensed by the State of New York
- A Bachelor's degree from an accredited college or university or an Associate Degree in Nursing with 2 years' experience
$85,000 - $115,000 per year, depending on experience
