We are seeking a highly skilled Patient Care Coordinator to join our team at Northern Light Eastern Maine Medical Center. As a key member of our Urology Clinic, you will play a vital role in ensuring the seamless coordination of patient care services throughout the continuum.

The successful candidate will serve as a consistent care coordinator for diagnostic and surgical end of urologic care, interfacing with schedulers, internal and external providers, and assisting with diagnostic imaging, clinical evaluation, support services, and education of patients receiving services. This is an exciting opportunity to make a meaningful impact on patient outcomes and contribute to a collaborative healthcare environment.

About the Role:

  • Develop and implement triage standards to include proper telephone etiquette
  • Collaborate with other disciplines based on patient assessment and identified needs
  • Complete assigned or requested tasks in a timely, efficient manner
  • Coordinate patient activities within the department and/or among departments
  • Actively work to creatively problem solve to meet service and program needs (patient, family, staff, and physician)
  • Provide day-to-day supervision of clinical operations and personnel performance
  • Assist in coordinating educational opportunities for staff development
  • Provide outreach and community education as requested
  • Develop and maintain through effective collaboration a competency-based orientation program
  • Coordinate the development, implementation, and evaluation of patient/family education programs

Requirements:

  • Registered Nurse (RN) license in good standing
  • Associate's degree in Nursing or related field
  • Minimum of 2 years of experience in a clinical setting, preferably in urology
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Strong problem-solving and critical thinking skills
