Healthcare Liaison Specialist

Arkos Health seeks a skilled Community Health Worker to join its team. As a key member of the organization, you will play a vital role in improving patient outcomes and delivering high-quality care. The successful candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. They will also be highly organized, able to prioritize tasks efficiently, and comfortable driving in the field up to 80% of the time.
Benefits include employer-paid medical, dental, and vision premiums, as well as short-term disability and life insurance. A comprehensive 401(k) plan with employer matching is also available. The company offers 15 days annual combined sick/vacation and 9 days of holiday during the first year, plus your birthday off.
Key Responsibilities:
- Enhance patient management and promote continuity of care through therapeutic interaction, supportive education, and advocacy.
- Develop and implement effective care plans that meet the unique needs of each patient.
- Cultivate strong relationships with healthcare providers, community organizations, and other stakeholders to ensure seamless care coordination.
- Provide education and support to patients and families to empower them to take an active role in their healthcare.
- Collaborate with multidisciplinary teams to achieve optimal patient outcomes.
- Document all patient interactions, including assessments, interventions, and outcomes.
- Participate in quality improvement initiatives to enhance patient satisfaction and clinical outcomes.
