Role Overview:


This position functions as a provider within the interdisciplinary team of Home-Based Medical Care (HBMC), directly delivering medical services to a designated patient population. Focused on enhancing the health and well-being of patients, this role collaborates with primary care physicians, HBMC team members, and other healthcare professionals to develop and implement individualized care plans throughout the patient journey.


Key Responsibilities:

  • Practices in accordance with state regulations governing advanced practice nursing.
  • Establishes and nurtures effective relationships with patients, caregivers, and interdisciplinary team members.
  • Conducts comprehensive assessments for newly enrolled patients and provides ongoing care, updating management plans as necessary.
  • Completes follow-up assessments in accordance with established protocols.
  • Prescribes necessary diagnostics and interventions to prevent unnecessary hospital admissions.
  • Demonstrates proficiency in essential nursing procedures, including IV placement, blood draws, and wound care.
  • Coordinates with hospital teams upon patient transfer notifications.
  • Educates patients and caregivers on disease management and wellness promotion.
  • Provides guidance and support to team members as appropriate.
  • Actively participates in meetings focused on patient care and clinical excellence.
  • Implements quality initiatives to uphold high standards of care and enhance care management.
  • Available for on-call duties during business hours and participates in after-hours on-call programs on a rotational basis.
  • Stays informed on relevant medical research and advancements through continuing education and professional development.
  • Performs additional duties as assigned.

Required Qualifications:

  • Completion of an accredited Nurse Practitioner program.
  • Current Nurse Practitioner license in the State of New Jersey (ANP, FNP, or GNP credentials).
  • Minimum of 2 years of clinical experience in primary care, home-based medical care, geriatrics, or emergency care.
  • Familiarity with long-term care and community resources.
  • Reliable transportation access.


Preferred Qualifications:

  • Knowledge of CPT, ICD-10, and HCPC coding.
  • Understanding of clinical care standards.
  • Awareness of NCQA requirements and CMS guidelines.
