We are seeking a skilled Physical Therapist to join our team at JAG Physical Therapy. As a licensed therapist, you will be responsible for assessing patients' strengths and weaknesses, creating tailored treatment plans, and ensuring patient confidentiality while fostering a positive environment.

You will work closely with an interdisciplinary team to provide comprehensive care and education to patients.
Responsibilities- Collects and records patient care data, including initial evaluation, daily notes, reassessments, progress reports, discharge plans, and communication with the team.
- Prepares for evaluations, obtains patient history, and researches unfamiliar diagnoses.
- Selects and administers tests and measures, synthesizes evaluation results, and formulates a diagnosis, goals, prognosis, and plan of care.
- Establishes treatment plans based on evaluation results, goals, and best practices, collaborating with the team, patients, and their support system.
- Provides interventions to patients, including therapeutic exercises, joint mobilization, and modalities, while maintaining a safe and effective treatment environment.
- Maintains accurate and timely documentation, including Prompt EMR note writing for all treatments provided.
- Oversees support staff, including PTA, ATC, and PT aides, promoting a culture of teamwork and collaboration.
- Bachelor's or Master's Degree required, Doctorate degree preferred.
- Licensed Physical Therapist in the state practicing in.
- Completed state-regulated CEU requirements in the appropriate state (NY, NJ).
- Valid CPR License.
- Experience/knowledge with EMR software is preferred.
- Excellent oral and written communication skills.
- Strong clinical decision-making skills.
- Basic computer skills.
- Maintains a positive, professional demeanor at all times.
- $120,000 - $150,000 per year salary.
