Nurse Care Specialist

We are seeking a compassionate Registered Nurse (RN) to join our team at The Commons. As a Registered Nurse, your main role will be to deliver high-quality nursing care to our residents within a warm, comfortable, and home-like environment. You will be part of a team of dedicated, like-minded individuals who support and care for one another, building a sense of community and long-lasting relationships.
Responsibilities:
• Accurately assess, communicate, and document residents' status, ensuring their needs are met and their well-being is prioritized.
• Observe and report any significant changes in resident behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director, adhering to state regulations and guidelines.
• Direct and supervise the daily work assignments of the Resident Care Assistants, providing guidance and support as needed.
• Mentor and guide the Lead Resident Care Assistants in all aspects of their job, promoting growth and development within the team.
• Coordinate care needs with community providers through an effective case management process, ensuring seamless care transitions and resident satisfaction.
• Effectively balance service demands with supporting resident independence, promoting autonomy and dignity.
• Recognize and assist in the prevention of elder abuse, neglect, and exploitation (financial and other) and report to appropriate sources per state regulations.
• Respond to emergencies and personal emergency response system, knowing when to call for backup and ensuring resident safety.
Requirements:
• Current state license as an RN and CPR certification, ensuring up-to-date skills and knowledge.
• Graduate of an approved RN program (per state requirements), demonstrating a strong foundation in nursing principles and practices.
• Minimum of 1 to 2 years of experience working as a nurse, with a preference for experience working with the elderly in a paid or volunteer position.
