Health Services Program Manager

We are seeking an experienced Care Coordination Expert to join our team as a Health Services Program Manager. This role will play a critical part in ensuring the medical needs of our clients are met while working closely with internal and external partners to provide seamless care coordination.
The ideal candidate will have a strong background in healthcare and experience working with vulnerable populations. As a Health Services Program Manager, you will be responsible for:
- Developing and maintaining expert knowledge of regulatory requirements pertaining to medical needs, guidelines, laws, and best practices in the supported living field.
- Working collaboratively with the WA Residential Director to ensure clients receive high-quality service and their needs are met.
- Re-train DSPs as needed when medical errors occur or trends are noted.
- Communicating with the Nurse Delegation Team, DDA, and medical professionals regarding individual care, including written and verbal correspondence.
- Investigating incident reports involving clients, interviewing staff to collect necessary documentation, creating detailed reports, and scheduling meetings with the Regional Director and department director.
- Generating reports and providing recommendations during leadership meetings regarding incident reports and behaviors.
- Managing Nurse Delegation by ensuring clients requiring delegated tasks are scheduled for required visits, documentation is received and up-to-date, and qualified staff are delegated to complete delegated tasks.
- Maintaining medical records as required by regulatory guidelines.
As a Health Services Program Manager, you will be working in a fast-paced environment that requires multitasking, working under pressure, self-direction, and frequent interruptions. You must be able to lift and/or transfer a minimum of 50 pounds and work in various settings, including client homes in multiple counties and cities.
The estimated salary for this position is $67,724.80 + On-Call Stipend, and we offer a comprehensive benefits package, including medical, vision, and dental insurance, voluntary short-term and long-term disability, and a 401(k) plan.
