Job Overview

This role serves as an internal resource for community referrals and linkages, participating actively in the delivery of client services and coordination of community support services.

The Case Manager will develop and maintain relationships with other community agencies and organizations, requiring good character, understanding, physical health, emotional stability, and personality suited to meet the physical, mental, emotional, and social needs of the population served.

Key Responsibilities
  • Establish liaison relationships with community resources, providing outreach service to clients.
  • Demonstrate professionalism and appropriate boundaries in all interactions.
  • Work within schools or other non-traditional settings, and work from multiple locations.
Requirements
  • An Associate's Degree in Behavioral Health, Health Sciences, or a related field, and two years of closely related work experience is required. Appropriate work experience may be substituted for the degree.
  • Experience in Mental Health or Case Management preferred.
  • Excellent verbal and written skills are a must.
  • Must be eligible for Agency Affiliated Counselor Registration from the State Department of Health.
  • Will manage cases and contact families, if necessary make home visits.
