About the Role

We are seeking a highly skilled and compassionate Professional Patient Care Coordinator to join our team at OU Health.

Job Summary

The Professional Patient Care Coordinator will provide social services to patients and their families, ensuring they receive the necessary support and resources during their hospital stay. This role requires strong interpersonal skills, analytical thinking, and the ability to explain complex information in a clear and concise manner.

Key Responsibilities
  • Conduct discharge planning evaluations to identify patients' needs and develop personalized plans for post-hospital care.
  • Make psychosocial assessments and collaborate with hospital staff to ensure comprehensive patient care.
  • Counsel patients and families on various topics, including disease management, treatment options, and emotional well-being.
  • Identify high-risk patients and provide crisis intervention services as needed.
  • Serve as a liaison between patients, families, and healthcare providers to facilitate communication and resolve issues.
Requirements

To be successful in this role, you must have a Master's Degree in Social Work from an accredited institution and relevant experience in a hospital setting. A Licensed Master Social Worker (LMSW) is preferred. You should possess excellent analytical and communication skills, as well as the ability to work effectively in a fast-paced environment.
