**Job Description**

This opportunity involves conducting in-home health and wellness assessments for health insurance plans. You will review medications, perform physicals, check blood pressure, and collect urine samples. The assessments typically last 45-75 minutes and can range from 5 to 8 per day.

**Requirements and Qualifications**

To be successful in this role, you must have:

  • a valid NP license in the state where you will be working
  • an unrestricted state license
  • a CAQH and NPI number

**Additional Information**
