Job Summary

We are seeking a skilled Patient Flow Manager to oversee the day-to-day operations of our inpatient care management system across all Alameda Health System facilities.

About the Role

As a key member of our team, you will be responsible for managing patient flow, ensuring efficient care coordination, and developing strategies to improve patient outcomes. You will work closely with our Vice President of Care Management, Physician Advisors, and other stakeholders to achieve our goals.

Main Responsibilities
  • Develop and implement case management policies, practices, and procedures aligned with regulatory requirements and hospital policies.
  • Collaborate with cross-functional teams to optimize care coordination, reduce readmissions, and improve patient satisfaction.
  • Analyze data to identify trends and areas for improvement, and develop actionable plans to address these issues.
  • Provide educational support to staff on case management best practices, InterQual criteria, and resource management.
  • Serve as a liaison between Alameda Health System and community agencies, primary care providers, and other stakeholders to ensure seamless care coordination.
Requirements
  • Masters degree in Nursing or Social Work, or related field.
  • 7-10 years of case management experience, including progressive leadership roles in large healthcare organizations.
  • Active licensure as a Registered Nurse in California or valid license as a Clinical Social Worker.
