About the Role

We are seeking a compassionate and skilled Patient Care Specialist to join our Hospice Aide team at Beaumont Home Health and Hospice. As a key member of our care team, you will provide personalized support to patients in their homes, promoting comfort and dignity throughout their journey.

Job Summary

The successful candidate will be responsible for delivering high-quality patient care, adhering to industry standards and best practices. You will work closely with our interdisciplinary team to develop and implement personalized care plans, ensuring seamless coordination of services and effective communication with patients, families, and healthcare professionals.

Key Responsibilities
  • Provide personal care and assistance with daily living activities, including bathing, dressing, grooming, and toileting;
  • Assist with meal preparation and feeding, as needed;
  • Support patients with mobility and exercise routines, under the guidance of our therapy team;
  • Maintain accurate records of patient interactions, progress, and any changes or concerns;
  • Collaborate with our nursing staff to identify and address patient needs, adapting care plans as necessary;
Requirements
  • Valid driver's license and reliable transportation;
  • Ability to lift, transfer, and position patients with care and precision;
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and colleagues;
  • High school diploma or equivalent required; one year of experience in hospice care or a related field preferred;

Estimated Salary: $45,000 - $60,000 per year, depending on experience and qualifications.
